Pilot privacy notice
Last updated: October 28, 2026
Who we are
CertDesk is a product of Apollo5 LLC, 1255 N Hamilton Rd #1032, Gahanna, OH 43230-6785. On this site, CertDesk means certificate-of-insurance tracking for general contractors and property managers.
What this notice covers
This notice covers only the pilot request form on this site. It does not cover a CertDesk account, uploaded certificates, or the product application.
What the form collects
If you submit the form, we collect:
- Your name
- Work email
- Company
- Role (general contractor, property manager, or other)
- About how many vendors send you certificates of insurance (Under 50, 50–100, 100–300, or 300+)
- How you track those certificates today, if you tell us
- The time we receive the submission
- A random request key created in your browser, so a second send of the same request does not add another row
The form also has a hidden field meant to catch automated spam. If that field is filled in, we drop the submission and do not add a row.
We do not ask for a certificate, a policy number, or payment details on this form.
Why
We use the answers to reply by email about the pilot and to learn whether the product fits your setup. Under 50 vendors is still useful. A free tier might already suit a shorter list.
We do not use the form to decide whether anyone's insurance is adequate, and we do not mark a vendor compliant from it.
Where it's stored and who processes it
When you submit the form, your browser sends your answers to a Google Apps Script web app, which adds one row to a private Google Sheet in Apollo5 LLC's Google Workspace account. Google Cloud hosts this site, and Google Workspace handles mail sent to support@certdeskcoi.com. Like any web host, these services record technical data such as your IP address, browser type and the time of the request to run and secure them. These providers get only what they need to do their work for us. Information is stored in the United States. We don't sell or rent personal information, and we don't share it for advertising.
How long we keep it
We keep a pilot request for 12 months from when we receive it, unless it becomes a customer account. You can ask us to delete it sooner. If you opt out of email, we keep only your email address on a do-not-contact list so we don't contact you again. That list is shared with webada, another Apollo5 LLC product.
Cookies and other tracking
This pilot site does not set cookies and does not load analytics, ads, or third-party fonts. The form request goes out only when you press the button. Until that request is confirmed, this tab keeps the request key in its session storage so a retry is recognized. The key is not a cookie, and it is removed after a confirmed save.
Your choices
To see, correct or delete your pilot request, or to stop email from us, write to support@certdeskcoi.com with the name and work email you used. We'll respond within 30 days. You can also reply "unsubscribe" or "no thanks" to any email from us. We act on opt-outs promptly, and always within 10 business days.
